Letter of Intent to Homeschool

This is a general letter of intent to homeschool that is as a courtesy only. It is not mandated by law to turn in to your principal or school board, however, many choose to do so.

This is a sample transcript that you can use to create your own.  The classes listed are just generic classes and do not reflect any official course listing.   You should fill it in with your own information

Private School/Church School Enrollment ​(Superintendent) 

Your private school or church school enrollment form is due to your local district superintendent at the initial decision to home-school. If you are enrolling in a traditional private school or a church school, your administrators will most likely complete this step for you. Driver's License Homeschool Affadavit

Proof of enrollment is required to get a license.  

Driver's License Enrollment/Exclusion Form

This form is used by provate/public/cover schools, but can be used by homeschoolers.  To complete, simply fill in the name you have given your home-school under "Name" of Section II and select "Secondary School" in the box on the right. 

ALDSE Public School Graduation Requirements

These are requirements for public school students only and do not apply to home-schooled students. This document is included for reference only.
